If you don't have a vet prescription yet then you can download a blank Prescription Form here which must be filled in and signed by your vet before posting to us.

* Prescriptions are only valid for six months from the signed date or as stipulated by the prescribing vet, whichever comes first. Prescriptions for Controlled Drugs such as Epiphen are only valid for 28 days.
Repeat Prescriptions for Long Term Conditions
For long term conditions which are stabilised on medication, your vet may be prepared to provide a repeatable prescription. This can be repeated for as many times as stipulated on the prescription within six months from the date of issue. If this applies to your pet, it may be worthwhile requesting a repeat prescription which can save you making monthly visits to the vet for a new prescription. However, it's at your vet's discretion whether a repeat prescription is appropriate for your pet.
